Been there.

To identify where the problem was I placed it back on track tightened everything up then would check the window up/down to see if anything was restricting or causing it to pull off track. After pinpointing where it was coming off track I decided to use a larger head bolt to keep it on track. With mine it was off track due to the head bolt just slipping through, after using a larger head bolt it never popped off again. The bolt was the same size, only the head was larger with more surface preventing it from sliding around on me after tightening. Give it a try it may help you out.

Use lithium white grease to oil the rubber tracks after as well to prevent too much resistance possibly pulling the window off track.
