Any CEL codes?

Is this a low idle problem or are there driveability/running issues?

It sounds like you're only idling during fast idle, which leads me to believe that you aren't getting enough air during normal idle. Check/clean your idle valves and TB. Properly set the idle, preferably using a shop manual. If you are unable to fix the problem with the above suggestions, try a compression test.
