If it's anything like an 02 bumper, there are four pop-out fasteners near the hood release latch at the radiator, one screw in each wheel well (turn the wheels in opposite directions for better access), and screws (and/or fasteners) scattered along the underside of the front bumper attached to the splash guard; I want to say there are 8 or so of them.
After you remove the above fasteners and screws, the bumper should be held onto the front fenders by plastic clips. Pull gently (outward) on the bumper where it meets the fender and it should release.
I recommend placing cardboard or a towel underneath the bumper because it might slam down into the ground after you pop it away from the fender (since nothing else should be holding it up). Also, if you have fog lights, be careful to not yank the bumper away before unplugging the harness from the bulbs. Also note the grill might catch the radiator cap...so keep an eye on that.
Reverse to install.
I hope I didn't miss anything...if I did, please correct me.
Edit: In terms of reattaching the bumper with new screws, it depends on why your bumper is loose and falling off. I couldn't imagine it falling off unless the tabs on the fender are broken and the screw in the wheel well is missing; or the fasteners at the radiator are broken and missing.
