Idle fluctuations are most commonly caused by FITV, IACV, or an improperly bled coolant system.

To remove air from your coolant system run the car on a level surface with the radiator cap removed until the fans cycle on and off several times.
Keep the radiator topped off with coolant while doing this.
On the 5th gens there is also a coolant bleed valve on the thermostat housing to help speed up this procedure, I believe it is on the 4th gens as well but I'm not for certain.
