whats this elbow and flange that comes off the wastegate? what goes to it?

your turbo manifold it might be for a blox or inline pro manifold or drag manifold people use it for valve cover or hood clearance

i guess if someone didnt want the air to go straight out, they could change it.. does it matter?

Yes, you can actually gain power by using a properly routed dumptube. Think about it, you don't want your turbo suckin up that hot exhaust now do ya.
