The problem is obviously the gap on the TB. You shouldn't be seeing through it like that. Does the Holley have a provision for adjusting the throttle stop? I know on many cars I've worked on I've had to back down the stopper a bit to make sure the throttle closes all the way.
The reason it idles fine when you unplug the IACV is that the ECU is adding extra air from the IACV on top of the air from the air leak through the throttlebody. When you unplug it, it brings the flow down lower to where it would need to be to get a smooth idle. You could achieve the same effect without a CEL by blocking the IACV port or closing the IACV screw all the way, but that's not really fixing the problem.
The fact it's surging is there's a zero throttle rev limit in the Honda ECU, it's something like 1500 rpm. If you adjusted the TPS the other way so it thought it was getting throttle all the time, the idle would probably be closer to 2500 or 3500 rpm.

I had the same exact problem when I put my Edelbrock 65mm on my JRSC LS. I was leaking under where the TB boltedup to the S-tube. I pulled it off and there was a "extra" port (if you wanna call it that ) on the bottom of the TB for what ever reason that my JRSC S-tube did not like. I filled it in with JB weld and installed....I have no problems at all now. Take a look at the bottm side of the TB as compared to the stock TB...that may be it. Good Luck!!!

Remove air intake hose from Tbody. Plug the hole leading to the butterfly with something (airtight). If it idles ok after that, you know the Tbody is at fault.
Do you tune the car yourself? In most tuning software, you can influence the IACV. Surging idle can usually be fixed by reducing the IACV setting.

Alot of guys sware by the RPM services TB. Don't get the skunk2, the throttle sticks.

its the stopper... i dont rem if that tb exactly has it but if i remember correctly it does... adjust the stopper and readjust your tps and all will be fine.
