Hi!
I have problem with my b20 vtec.

loosen the tensioner and move the belt outward until it is flush with the outer edge of the cam gears, tensioner, water pump, and crankshaft. its that simple

did you move the belt outward to flush on ALL gears/pullies? if you only make it flush with the cam gears, the tensioner from the crank/water pump/etc walk right it back in. if you did make it flush with all gears, then i would think something is bent or not torqued down. maybe a bent cam gear or tensioner. i would get a new belt as well. that one looks damaged.

That's pretty strange. It looks like a bit of a line on the tensioner pulley. I'd put a new tensioner and belt on it and try it again. But first, post up a vid of the belt running on all the pulleys?

I'd lean toward over-tightening the belt and the tension plus the spin of the motor pulling it inwards towards the block.

I tried b20 and b16 crank gear, oem pr3 cam gear and adjustable cam gear, but nothing. I try another belt and tensioner.

Also make sure the belt guide on the INSIDE of the crank gear is installed with the smooth-rounded side facing the belt. I can't tell from your pics if it is... the front side looks correct. I agree with 1HGEJ2... replace the tensioner and bolt along with a new belt and try again. If there is an alignment issue, moving the belt closer to the outer edge of the gears as suggested by some above will not FIX the problem... it will ride there for a few revolutions and then move inward on it's own.
