idling/ sputtering/ bad ground?

idling/ sputtering/ bad ground?
-LS b18 obd0, stock everything, pr4 ecu
-constant alternator whine through my subwoofer, subwoofer ground is good.
-static "pop" noise from the sub whenever i use blinkers, turn lights on, or use press any buttons/ switches on the steering column or stereo.
also, when i cold start my car, it will idle high at about 2000 rpm, and occasionally bog and sputter down to 1000 for a few seconds before going back up. otherwise it will barely run at like 100 rpm for a few seconds and die. seems to run rich.
as soon as the car is warmed up (10 min), it idles perfectly at around 800 rpm, with no problems idle or drive-ability wise.
the popping noises and whining thru the subwoofer are always present though, and today i noticed when my headlights are off, the engine idles higher, and once i switch them on, it dips down a good 250 rpm.
so i feel like all of these problems are related to a bad ground somewhere? but where? my only code is 43 fuel supply, and it sparingly comes on, with no changes noticed in the performance of the engine whether it comes on or not.
