Default 98 Honda Civic LX overheating.

So I am having an overheating issue. Two days ago my check engine light came on and my car started to overheat. When I had stopped it and popped the I saw that my upper radiator hose had a large rip in it. I replaced hoping that my problem would be solved but it has not. The car still overheats. From the time I start the car the radiator fan would kick on, provided that the AC was on. Now it turns on for about ten seconds and then turns off. It repeats this the entire time the car is running. Also, while the heater is running the temperature drops about halfway but as soon as I turn it off or turn the AC on, it shoots back up to higher end of the temp gauge.
Any thoughts?

Full on coolant?
Check or replace thermostat,
Bleed all air from system,

Other possible water bump issue. Not likely but possible.
