Cause for v6 05 accord timing belt slip off?

Rod cracks after reman
Had to alter this post because after initially thinking the timing belt slippe doff after a full reman on an 05 v6 accord, it was actually a rod that cracked.
What could have caused this? Is this sloppy work on the part of the engine rebuilder or just bad luck???

Had a timing belt tensioner freeze up on a timing belt with only 20k on new belt but old tensioner.
Engine totally remanned and after a couple hours the new timing belt slid off AGAIN ruining the engine.
What could be causing this?
Luckily it is covered but I am starting to think this car is jinxed. Yes, it is a reputable engine shop.

actually, found out it was not the belt, the Rod broke and cracked the case. Reman guy has to do everything all over again.
