As some have pointed out, your sleeve install is definitely not the best. The holes between my cylinders lined up almost 100% perfectly. HOWEVER, your headgasket is OK. That's how the holes line up...

My 46mm 2" dump into modified Full-Race 3" dp. I couldn't take the noise anymore either and finally whipped this up. Will never go back, it's much nicer quiet.
