Default transmission

I am having problems with my transmission skipping a gear after I changed my transmission fluid and cleaned the transmission, why is that, do I need a new transmission

Bring engine/transmission up to normal temp as indicated by the temp gauge.
Place the trans in each gear for a few seconds, R>N>D4>D3>D2>D1. Then place it back into Park. Remove the transmission dipstick and wipe it off, reinsert fully and pull it back out, is the fluid with-in the hash marks? If not fill until it is with-in the hash marks while the engine is running.

When changing fluid on an older trans the tolerances become a bit bigger, and the wear from the clutches requires slightly more fluid than what is normally scheduled for service.
Also when changing the fluid if once allows the trans to 'drip' dry, the fluid inside the passages of the trans may be siphoned out when the plug is pulled.

Always charge each gear after a fluid change and always recheck fluid level.

What color was the fluid when you changed? I assume it's a 91 accord automatic. How many miles on the vehicles transaxle?

OP needs to give more info.

When was the last time the fluid was changed? Car mileage? What type of fluid was in the car before and what type did you add now? Did you do a drain & fill or have a shop power flush it ? Is the CEL on and/or stored codes present? Was the tranny slipping before the fluid change?

The car had 216,000 miles on it. I changed the transmission fluid because it was Brown and I know it's not suppose to be that color.

I first used Lucas transmission cleaner, ran that for 3 miles to get the gunk out and then drained that and put the Carroll multi vehicle import transmission fluid back in, but it was doing this before I did the fluid change.

But it was going that before I used the Lucas seafoam for transmission

For Hondas, they are very finicky about tranny fluid, whether auto or manual, mostly manual. I would get some oem fluid and swap that in instead.

Usually, when the transmission color is brown and does not smell burnt it is best to leave it be. The brown color is the clutch material suspended in the fluid. Once you flush that out you will lose a significant amount of friction between the clutch plates, and sometimes that may be the ONLY friction material between the steels and clutches

Never use a 'multi-purpose' ATF in any transmission. If one type of fluid could work in all different transmissions we would not have Type F, ATF +4, Dexron II;III;IV, Mercon V etc etc.

Use Dexron II(embossed on the rubber plug of the dipstick) at a minimum, most automotive stores will have this in a gallon jug.

If the transmission was slipping before, changing the fluid would not have fixed the problem.

If the transmission is simply missing/skipping a gear then there may be an issue with the shift solenoids on the side of the transmission.

Does the D4 light work? If so does it blink? If so have you pulled the codes via the two wire blue connector over above the kick panel? If so what was/were the code(s) if any? If no codes are present the shift solenoids may be gunked up or the screens may be blocked preventing a proper shift event.

Unhook the electrical connectors to the shift solenoids. With a long enough jumper wire, directly place +12 volts from the battery to the unplugged solenoids pig tail, do the solenoids actuate with a swift 'click'? If so then the screens may be plugged. Remove the solenoids, pull the filters off and clean them, using a Q-tip shaft make sure the solenoid is not gummed up with junk.
Reassemble/Reinstall and see if the transmission shifts.
