Default P0141, P0171, P0303, P0304, P0401, P1300

I have a '96 Accord EX. It is carefully maintained and has been running well. Suddenly the Check Engine Light comes on, so I go to my friendly, neighborhood Autozone to have the codes read. To my surprise, there are six:
-P0141 Oxygen Sensor Heater Circuit Malfunction (Bank 1 Sensor 2)
-P0171 System too Lean (Bank 1)
-P0303 Cylinder 3 Misfire Detected
-P0304 Cylinder 4 Misfire Detected
-P0401 Exhaust Gas Recirculation Flow Insufficient Detected
-P1300 Multiple Cylinder Misfire Detected

I put on a new down-stream 02 sensor, drove for a while, and the Check engine light went out. So, I took to for its Illinois Emissions Inspection and it passed with flying colors. After that, I drove it a couple more hours and the Check Engine Light came back on!!! Then, I took it back to Autozone to have the codes read again. The codes are the same as before.

Any ideas what's going on here? Can one of these problems trigger all the other codes? Any advice will be appreciated.

If the EGR ports are plugged up it can cause a 'lean burn' misfire.

But with a new O2 sensor I don't know why the heater circuit recoded.

Were the codes ever cleared the first time around?

Was the new o2 from Auto zone?, if so throw that crap in the trash and get a Denso from Honda ..
How many miles on your ride???? if less than 150K take it to the dealer and they should take care of you problem due to an emission extension warranty
if not , clean your EGR ports (very easy with just a few tools) becarful with the fuel lines and injectors..
New Honda secondary O2 sensor
Valve adjustment(for good measure)
Check your plugs, wires, cap &rotor..(again if less than 150k honda provides special warranties for these parts) Service Bulletin 98-081
