Car stays in 2nd gear
1996 Honda Accord DX 2.2 - only drives in 2nd gear (limp mode).
Did some major repairs. Engine work, new torque converter. Did not bleed the coolant. Ran okay at first. Believe the engine/tranmission got hot because there was air in the coolant. Some fluid came out on the ground, believe it was transmission fluid. Now it will not shift out of 2nd gear when driving it.
Tester set off error codes for Sylenoid B & speed sensor. The wiring was loose on sylenoid b, the speed sensor is brand new. Took it back, error codes went away but then it showed Transmission Range Sensor Circuit Malfunction 2-posi. Inspected it, and it looks fine, the car shifts into any gear, just won't shift when driving, no problems starting it up. Starts up in any gear but after you put the key in, you push a button to start it up. Does that make a difference? Should it not start up in any gear?
Any ideas what could be going on? Could the torque converter be faulty?

It should only start in Park and Neutral.
What was the actual OBDII code?(P07** or P17** format)
What is the D4 lamp code? (jumper two wire blue connector under/behind glovebox door, turn key to II(ON) and count the blinks from the D4 lamp)
Fix the wiring issue.
Find out what was done to install the pushbutton start.
If the gear selector switch inside the console is broken/damaged the car may be able to be started in any gear, but the TCU will not know what gear you have selected for correct shifting.

Found out the throttle body was soaked in purple power & the TPS was left on it? Wouldn't that ruin a sensor? Could that be the reason the car is in limp mode && would it cause symptoms with the engine because it is fine other than the car running in 2nd gear only.
