Default Honda Accord - Transmission Issues In What Years?

I'm looking at early 00's Honda Accords, but keep coming across warnings about faulty transmissions that give out repeatedly even at lower mileage. It looks like it definitely affects the 1999-03 models (based on various complaint sites, such as www.carcomplaints.org). It's not clear if the 04 is affected or not - or if it's just not old enough to have accumulated a significant number of feedback.

Does anyone know in what year an adjustment or update to the transmission was made to solve the problem?

Also, I read something about a transmission recall by Honda, but I'm not clear what it covers. Is this related to the same problem? If so, why does it only cover the 03-04 Accords and not the earlier ones that also had issues?

Just wanted to add, I have searched the forums and found some posts that say it was the 98-02 years, but that doesn't seem like it's strictly true looking at the issues here: http://www.carcomplaints.com/Honda/Accord/2003/ and here: http://www.hondaproblems.com/Accord/ . But I don't understand why the crap transmission would be kept into the 2003 redesign??

Summary:
ON SOME MINI VANS, SPORT UTILITY AND PASSENGER VEHICLES, CERTAIN OPERATING CONDITIONS CAN RESULT IN HEAT BUILD-UP BETWEEN THE COUNTERSHAFT AND SECONDARY SHAFT SECOND GEARS IN THE AUTOMATIC TRANSMISSION, EVENTUALLY LEADING TO GEAR TOOTH CHIPPING OR GEAR BREAKAGE.
Consequence:
GEAR FAILURE COULD RESULT IN TRANSMISSION LOCKUP, WHICH COULD RESULT IN A CRASH.
Remedy:
ON VEHICLES WITH 15,000 MILES OR LESS, THE DEALER WILL UPDATE THE TRANSMISSION WITH A SIMPLE REVISION TO THE OIL COOLER RETURN LINE TO INCREASE LUBRICATION TO THE SECOND GEAR. ON VEHICLES WITH MORE THAN 15,000 MILES, THE DEALER WILL INSPECT THE TRANSMISSION TO IDENTIFY GEARS THAT HAVE ALREADY EXPERIENCED DISCOLORATION DUE TO OVERHEATING. IF DISCOLORATION EXISTS, THE TRANSMISSION WILL BE REPLACED IF DISCOLORATION IS NOT PRESENT, THE DEALER WILL PERFORM THE REVISION TO THE OIL COOLER RETURN LINE. THE RECALL BEGAN ON APRIL 21, 2004, FOR PILOT, ODYSSEY, AND MDX OWNERS. OWNERS OF THE ACCORD VEHICLES WILL START RECEIVING LETTERS ON JUNE 28, 2004, AND ON JUNE 29, 2004, FOR OWNERS OF THE TL AND CL VEHICLES. OWNERS SHOULD CONTACT HONDA AT 1-800-999-1009 OR ACURA AT 1-800-382-2238.

All the problems were on V6 engine cars from 98 to early 04 models. There have been failures of 4 cylinder transmissions, but no more than any other make/model. I have an 03 V6, and the recall was done at about 15k miles. The transmission is still going strong at 67k miles, but I don't abuse it, and change out 3 quarts of fluid every 15k miles.
