EGR Lift Sensor - code 12

I scanned my car myself and this is the code i'm getting... LONG,SHORT,SHORT!!
which is a code 12?! EGR Lift Sensor!!

is that replaceable?! or fixable?! do i need a new sensor or is it something else?!
