choking/misfire while driving and at idle

choking/misfire while driving and at idle
hey guys here's my story...have a 97 cl 2.2 5-speed (same as an accord), over 150k miles with intake and muffler and nothing else modded.
while driving around 40mph, lights on the dashboard started flashing(like once or twice) then car started choking/misfire while rpm jumped up and down. then stalled/died. i waited for a while then started it back with no problem. next day drove it and choked a little and then later choked/misfired and died on the road. changed the plugs there, didnt help. was able to drive back home choking along the way but more so at redlights.
checked the codes and came out: P0141 o2 sensor 2 bank 1 and P0401 Insufficient EGR Flow. cleaned out the filled egr ports and egr cover. also cleaned the injectors plus added some injector fuel cleaner to gas.
1st test drive was a lot better but choked softer and not often. later in the day i took it to a friend to take a look at and NO choke/misfire the whole time. ran fine a couple of days maybe about 2-3 days then IT ALL STARTED AGAIN. cleaned out the ports again(they werent that bad at all). drove it again, same choking/misfire.
any help would be appreciated guys. is it too much carbon build-up?....ignition problem?...main relay??...distributor/cap/rotor?...egr valve it self?...help out. thanks for any info.
