Flush - If this is only for street use, every 30K miles or 2 years, whichever comes first. If you use your car in autocross or track events, then any time you're at one of those events, your fluid should have been flushed within the previous 6-8 months.
Bleed - Only when you have problems (e.g. soft pedal).

i like to do mine about once a yr to yr and a half.
i use a cooking baster to get all the old fluid out of the res, refill with fresh fluid (really like the Valvoline SynPower fluid), then bleed all 4 corners twice to get the new fluid down into the lines & calipers.

I don't race or anything, but I flush my fluid every year or so. It's kinda overboard, but I'm sort of **** when it comes to fluid maintenance. I've got a Vacula vacuum bleeder at work, which makes the process a breeze. I just suck out the fluid from the M/C, refill with fresh fluid, and then bleed each corner with the Vacula for about 5 minutes or so to draw the remaining fluid out of the calipers and lines. I just use the generic DOT 3 fluid we've got at work.
