Default Gear Grinding: What are the causes?

I've read countless threads on 2nd gear, 3rd gear grind etc.

What are the causes of gear grind before it happens?

How can you prevent gear grind?

shift smoothly, don;t shift unless you use the clutch, power shifting will **** up the syncrhos, and if you shift from 1 to 3 . . . just make sure you roll the shifter through 2 before you shift to third

i get a funny sound when i go from 4th to 5th...going to change out my tranny oil...maybe that will help
